At present, patients are accepted for treatment by Canadian physician or nurse practitioner referral only. Letters of referral may be faxed to (403) 284-9293. Due to medical/legal coverage, only Canadian residents can be accepted for treatment at the Regional Fertility Program.
Referring Pro-Tip: To help ensure a smooth and efficient process for your patients, if your referral to our clinic involves more than one patient, please include the demographics of each person being referred. If you are not the primary care provider for both/all patients, kindly direct each to obtain a separate referral from their own provider.
